JF&CS VNA has been proud to offer Medicare-certified home health services since 1985. Whether delivering services after a hospitalization or directly from a community setting, JF&CS is able to respond with an integrated continuum of quality care that allows patients to remain safely in their homes and in the community for as long as possible.
To be eligible for home health services, patients must be homebound and have a physician order for services. JF&CS can work directly with the patient’s physician to establish eligibility. If eligible, services are covered by Medicare and MassHealth. JF&CS can also bill most other private insurance and managed care plans.
Certified Home Health Services
JF&CS VNA offers a full range of home health services, including skilled nursing, rehabilitation therapies, certified home health aides, and medical social work. Our clinicians work with the patient’s physician to implement care and monitor progress.
Cellulitis/Wounds
- Wound care
- Provide specific wound interventions
- Provide education on causes and prevention
Cardiovascular Disorders
- Provide education on signs and symptoms of heart disease
- Assess cardiac / respiratory status
- Teach nutrition / weight recordings
- Provide therapy for ambulation and energy saving techniques
Gait Abnormality
- Evaluate home safety
- Assess equipment needs
- Provide physical therapy
- Teach exercise for balance and to decrease falls
Diabetes
- Management of Type 1 / Type 2 diabetes
- Teach nutrition / glucose monitoring
- Instruct on skin and foot care
- Assess for signs and symptoms of hypo / hyperglycemia
- Recommendation of adaptive devices
- Evaluation for balance and falls
Medication Changes
- Instruct on self-management and compliance
- Provide education on side effects and interactions

Home health may not be the only service an individual requires. Our clinicians work in conjunction with other JF&CS Senior Services, such as the Geriatric Care Managers from Your Elder Experts, Geriatric Mental Health, and Jewish Healing Connections. Our goal is to provide services for the whole person – physical, emotional, and spiritual.
