The Family Education Workbook is designed to help families develop a life care plan for their family members with disabilities. Developed in partnership with CJP’s Disabilities Housing Initiative, each section of the Workbook is filled with questionnaires and checklists developed to help families document their child’s current needs, think about options for their future, and the skills, services and supports the child will need in order to achieve those goals.

This easy to use workbook and reference guide provides information on topics such as:
